D.A.W.N. CONTRACT

Please read, print out, sign, and mail the following contract to the above address. If you have not paid through Paypal, please include your check, money order, or cashier's check for a 3 month ($79.95), 6 month ($129.95) or 12 month ($189.95) membership with this contract.

1. I represent that I am disabled or interested in dating a disabled person, over 21 or co-signed by my parent or guardian, unmarried, and that all information I provide D.A.W.N. (including all information on the profile and questionnaire) is accurate, true, and complete. D.A.W.N. may terminate the contract without the refund of any monies in the event that false information has been provided by the client.

2. The client understands and agrees that D.A.W.N. is not responsible for false or misleading information supplied to it by the client or others and holds D.A.W.N. harmless from any actions resulting from any such false or misleading information.

3. The director of D.A.W.N. reserves the right to use any or all data supplied by the client for referral purposes. Profile given will include first name and phone number, unless client opts for three day period to decline referral.

4. D.A.W.N. is not responsible for the conduct, acts, or omissions of any of its clients arising out of an introduction or referral provided. D.A.W.N. makes no implied or expressed warranties other than its agreement to provide referrals as specifically set out in this contract and the client acknowledges that there has been no representation, promise, or agreement other than as expressly set forth in this written contract.

5. The client specifically authorizes D.A.W.N. to refer the client to any other client of D.A.W.N.. The client waives any and all claims, damages, losses, or causes of action against D.A.W.N. arising out of any referral of the client to any other client of D.A.W.N., and the client agrees to indemnify and hold D.A.W.N. and its personnel harmless from any and all claims, liabilities, damages, and expenses (including attorney's fees) whatsoever resulting from any introduction provided the client.

6. Initial application fee is non-refundable.
